Print Summer Camp Enrollment Forms

Print Fall Enrollment Form

Children 2 - 4 years of age
Make an appointment to visit Smart Toddlers ™ with your child.
Complete the appropriate Summer Camp or Fall Registration Forms
Submit the registration fee and first month's tuition.
An immunization record (Form 3231) must be on file for each child.

NOTE: If the session is full, we do maintain waiting lists.

We are currently accepting applications for 2010 Summer Camp and Fall 2010/2011 Fall session.
Summer Registration Fee: $50.00 non-refundable fee
*Summer Tuition: $420.00 per month (Full Time Schedule - Review Summer Camp package for available options)
Fall Registration Fee: $100.00 non-refundable fee
*Fall Tuition: $420.00 per month

Checks are made payable to Smart Toddlers and are due by the first day of each month. Any payments received after the 3rd of the month will be subject to a late charge of $10.00. All fees must be paid by the 4th of the month or your child will not be allowed to return to Smart Toddlers until payment is made in full. An additional $30.00 will be charged for all checks returned due to non-sufficient funds.

A thirty-day written notice is required for withdrawal from the program. Payment is expected for only the 30 days.
